Early modern, not quite mediaeval. It's from Ragusa, a tiny independent city-state on the Dalmatian coast, in what is now Croatia (the city is now better known as Dubrovnik). It depicts Christ standing on one side, and St Blasius, patron saint of the city, on the other. The denomination is a grosetto, and it dates from sometime around the 1600's. Example on BeastCoins.
Due to the religious nature of these coins, many were holed and turned into religious amulets. Finding one without a hole is particularly challenging.
