NGC probably has the ability to fabricate slab inserts based on the diameter of a coin. As for that error getting out of the Mint, it was probably found in an old mint bag(discontinued circa 2000). Since the discontinuation of mint bags, errors such as that have been even rarer.

they probably just used a half or silver dollar insert, Looking at the picture of the whole slab it looks like it would be the size of the half dollar, I don't think its big enough for a silver dollar
