It looks like a mad that is possilbly also a partial collar. Is the diameter larger than it should be on either side?

Just slightly larger. Coin is also bent. If layed face down on a flat surface, one can see a space between coin and surface of desk.

Looks like a combination of a minor off-center strike and a slight misalignment in the same direction.
