Medallions were made and primarily offered at the vatican as gift/souvenirs. There were limited pieces made by private artists as well, but most of those were medallions only, not cast with a loop to be worn as jewelry. They were primarily cast in Bronze or silver, and John was Pope from 1978 to 2005.
I have limited knowledge in this area and I would guess this piece is cast in Bronze from the early to mid 80's. Someone else may be able to chime in with more specifics to this actual piece.

Tracking down the specific origins of religious medallions like this is a near-impossible task, as it could have been made and sold virtually anywhere, not just in Rome.

There seems to be a name at the base of the Pope's portrait - that's probably a designer or maker's name. Can we get a zoomed-in pic of that area, please? That might be your best clue as to its origin.

yea ill try to upload a pic, but I did look at it under a magnifying glass and the letters are abit to scratched on the sides but in the centre its abit better, I could make out AMR but anything before that or after that I'm not sure. I have my geusses to what the letters are but the condition its in I dont think I can confirm it. sorry for the limited info on it my brother found it in the dirt while he was digging around
