Nevada Error In The Making?

Received this quarter in change yesterday. Also got 79 SBA near date, but we'll get to that later. Note the second flower down on right side. Normal flower shows 5 petals. This one the bottom left petal is just a small bump on the stem. Did this bump disappear and a 4 petal flower emerge?

After more looking, found this on other side. Die must be getting worn out. That big blob under the upper flower is susposed to be a naked branch.
