This has a part of cheek broken from die while it was made

The die has broken and the cheek & lips of the die ended up shoulder

amac44 - I don't think I understand what you are trying to point out?

The lump on the shoulder looks like some debris fell into the coining chamber and ended up sitting at one of the deepest points on the die face. The strike might then damage the die provided the object was hard enough - possibly a chip of steel or a small rock?
