Was thinking I would probably use a tooth pick to help remove the foam after soaking it of course.

Old black foam is brutal when it ages. I've seen it have an oily substance to it too that I've assumed was from the foam breaking down

Like type said, soak it in pure acetone for a dew hours and if needed lightly rub it woth a qtip while still submerged in a seperate soak after the first

I also thought it might be too far gone as well. I guess at this point it can't hurt to give it a bath and hope for the best.

The acetone cannot meddle with the metal of your medal (could not resist!).

Your medal cannot be hurt by this bath - even if it was in great shape. I have used acetone on hundreds (or more) coins for quite a few years now to remove any residues or organic matter (finger oils, etc) before putting the coins into 2X2 holders.

This is not always true. Many medals actually have an applied finish that can be stripped by acetone.
