This is a 1938 D Buffalo nickel from my Grandfather's collection. While it will never be sold and just passed down what grade would you all give it?

I do have a question concerning the back of the neck. The pitting that I'm seeing, is that from a fresh die? Every 1938 D that I've seen of PCGS GRADED 66 and above the back of the neck is smooth. Did the pitting come from a fresh die or is that from die wear? It seems that if a die starts to wear it would be smoother in that area. Am I wrong on this? I've seen plenty of Die Deterioration around the rim devises on coins but nothing around the bust.
