You have an example of what is called "Machine doubling." I refer to it as Machine damaged as nothing actually doubled, but reduced in size by die movement during the strike. The main thing to note on it the flat shelf like damage to the normal devices that alter them. It is common on most all denominations in varying degrees. It is not hub doubling (Which affects the dies) but damage caused by machine malfunction.
