My guess, is it's a Straight Clip, Struck Off Center, and a Lamination issue. Pretty neat error coin!

The strength of TRUST and weakness of the portrait is interesting, too. I'd love to see how a TPG would slab this critter.

Just based on the second to last image, it feels like the portrait and TRUST were added to the flaps with AI. How could the coin face have struck so cleanly on such a deformed piece of metal that would never fit in the collar?

One of the first thoughts in my mind was damage by the feeder fingers on its way out of the press. Not sure if the distance of the gouges works with the size of the feeder fingers though. I could be way off base with this idea.
