It pictures the Bell "Sigismund" (czech: Zikmund) from the Cathedral of St. Vitus St. Wenceslas and St. Adalbert at "Prague Castle" (czech: Pražský hrad).
The bell from 1549. It is the biggest Czech bell with a lower diameter 256 cm height 203 cm and the estimated weight of 16.5 tons.
I am not sure of whether this piece is a token or medallion and have not been able to find a reference to this piece in my little search, but hopefully this bit of information will help point you on the right path.
