It is a MAD. The reverse is the hammer die in this case. They changed the order of the dies a few years back. So we need to look at the reverse more for the MAD coins. It looks like a partial collar?
It can if the MAD is far enough off. The hammer die can hit the collar and force it down creating a partial collar strike opposite the MAD. This does not always happen though because the collar could just go down on the one side and keep the other side inside the collar.
MAD can happen on either side (or both sides) but the anvil die can not be very far off because the neck of the die is always contained within the collar. So the anvil die can be slightly "off centered" but not far enough to lose any of the design.
