It's coin counting/rolling machine damage, not an error. Most likely not something someone did intentionally.

its actually quite unfortunate, so many coins ruined. Not that they are rare or valuable coins, but hey...I just dont like seeing any coins in pain :P

You might have also noticed X marks on the obverse ( Queen ) sides of coins older than 70s...those are usually from ruthless coin roll hunters marking useless coins for either not being silver, not a rare variety or anything of value
