Yeah, I guess it should be in Canadian Coins but it really doesn't matter.

That's post-mint damage ( PMD). Maybe a punch of some sort applied to the coin.

Could be any kind of hollow punch as listed above..or maybe just a small piece of thin rigid pipe..
