I know nothing about coins but ran across this 1995 quarter. Not sure if it's been altered or if it's a mint error. There is almost no detail at all on George Washington's face, it's almost completely smooth to the touch. Any knowledge would be helpful...thank you.

That just looks like an ordinary well-worn quarter to me. Not sure where you're seeing the error.

The coin is just well-worn. This is very common and I don't know what error you are talking about.

PMD. It's strange to see a 1995 in such poor condition, but that's definitely not a mint error.

It looks to me like a pocket piece that someone constantly rubbed with their thumb. IGWT is in good condition, as is L-I-B-T-Y in Liberty. The reverse doesn't show nearly as much wear as the obverse. It may have been a worry piece.
