Looks like coin contact. Note the areas affected? You can see a wave of metal like a splash on those areas. So the coin was normal until the damage happened.

It took a well placed hit, causing the metal to get pushed over and flattened. It's Post Strike Damage (happened after it left the U.S. Mint).
