Help needed on this one... on the obverse side in the area of the neck, at first glance I was thinking die clash with that being part of the memorial from the reverse image. Feeder finger lines all over, and on the reverse side you can see the die clash in the normal area. Also, curious as to if possible DDR?
Just a normal die that did experience a die clash/clashes and has been polishing many times to remove it. When the fields were polished, the new/newer die scratches were made stronger. But the more they polished the die, the more the fields were reduced in thickness, thus reducing the size of the devices and the shallow die devices. But the more they over polish it, the more the bust can go away: Just a fact of life for a die. Eventually the die scratches flatten, but the reduced devices and bust will continue to be abused from this process of abrasion.
