2016 P Roosevelt Dime-Lamination Error Obverse?

Hi guys,

I have this 2016 P Roosevelt dime and would like to find out if this would be considered a lamination error on the obverse? A large chunk of metal is missing below Roosevelt's ear and a large chunk on the bridge of Roosevelt's nose.

Looks like incuse areas. Not sure what happened. Not showing a push out on the reverse?

Trying to see if they incuse marks are pushed out on areas that are already raised?
Can't tell for sure on the match up areas?

Looks like maybe struck through a couple clumps of gritty grease. I might set this one aside in a 2x2.

Yes someone else told me this also. Is it uncommon to have 2 strike thru's on one coin?

I have a hard enough time finding just one struck through on a coin so seems like 2 has to be pretty good -

I've never seen one like this either. They clearly came from the same source, though, so it's a lot more common than if you considered the two events to be independent.
