So I'm not really sure what's going on with this penn. it looks like it's been beat the heck and back scraped on concrete and then thrown in a shredder so if any of y'all can give me some info on how something like this could happen I'm just curious what it would take to do this to a coin.

Sometimes unless you were there, you may never know how a coin was
damaged but your explanation is as good as any.

So after doing some research I have learned that The Shredder and the Footclan had made short work of this Cop-er Coin :)
