The surfaces have been removed.
Also looks like you have lamination on the reverse.

I say worn down by something mechanical - not sanded by an abrasive - not that it reall matters though - PMD.

That is some form of mechanical damage - but does not change the value which is 5 cents.

What do you also think about the rim? It's not fully round it is like oval? you can see the edge looking straight on. The top and bottom. That is that from normal wear PMD also? IDK. Interesting to me. It can't be like this coin is an Die Adjustment Strikes? Just wondering. Thanks

If the nickel is oval shape, then it is damage.
Not close to a die adjustment strike.
