The reverse punch is showing on obverse side! Are you seeing same here? Should I grade this coin? Liberty as well looks a bit off as well as ear.

Not worth grading, but a nice die clash known as a prisoner cent.

Agreed, known as a die clash. And no, not worth grading unless there is some sentimental connection. It would cost you much more than the final value would be.
