Not an error, it is called Die Deterioration from worn out dies.
I believe in 1969 the mint produced new master hubs.

1968-D-Lincoln-Cent-Date-Error
It was caused by a swelling master hub for that year. Thus they created a new/new master hub in 1969, making the devices the normal size again. Note how the devices changed shape and locations on the design.
