This nickel has a strange make where the designer initials should be. Any ideas ?

It's just a worn die making the initials look that way.

Die Deterioration it is. Just normal signs of a die getting old. You will often see more metal flow lines, especially in the fields, as well as a lack of defined details in some areas, such as the initials in this case.
