Yeah, you are correct, it probably got stuck in a wrapper/counter machine at the bank.

Wowf! I have never seen a counter machine error so bold. It's so strong it's almost a genuine error! All kidding aside, I'd be tempted to keep it as a novelty item, at least for the moment. And to the forum!

It actually looks like there has been some lathe work attempted to the coin. It would take a long time for a wrapping machine to do that to a coin.

The coin has been machined.....the lines, etc. are too defined for a wrapping machine or counter to do that.
