Looks like a warm die, or weak strike, or PMD.
I only have a 10x loop but I can't see signs of this being ground down.
Figured I'd share to get opinions.

it looks like PSD to me. Nothing in the minting process could have caused this.

What stood out to me as weird is the B in liberty looked smooshed.
