I looked at the International Gramsci site, and a photo of him from 1935-1937 looks a lot like the picture on the medal. The reverse looks very similar to the regular Russian coin design, changed to remove the value. The piece could be from the mid 30's or later- it looks quite modern, but that style was used from the 30's in many places around the world. I would guess it was Russian, or possibly Italian after the War.
