Looks like it took a circulation hit. Looks too small to be a D mint mark.

I'm thinking circulation hit as well--looks to be approx. 50% of the size of the coin's mint mark.

There is no way during the striking of a coin that an extra letter or number
can suddenly appear ( with the exception of a dropped letter)
It simply can't happen . What you have is PMD.
