PCGS - Duke Karl August, son of Grand Duke Karl Alexander, graces the obverse of this 1894 medal. Struck in commemoration of his life, this gorgeous tribute is PCGS SP63 with none finer.

That could be it, but I have seen worse on an MS-65 Ike.

It's a gorgeous medal. It seems to me that the graders were tough on it.
Inordinately fascinated by bits of metal with strange markings and figures

They applied two full-step penalties, one for his poor posture and another for his untrimmed mustache.

In seriousness, though, the bullet hole in the forehead is distracting, and there are also dings on the left side of his mustache (our visual right), scratches on his cheeks next to the mustache dings, scratches on his left lapel, and a light but large semi-circular scratch in the right obverse field. I'm just guessing, but those collectively may explain the 63.
