Nice find. That polishing around Washington's eye socket is about the size of Lake Okeechobee.

This is not a dual misalignment. It's just an uncentered broadstrike with obverse collar clash. The design is offset in the same direction and distance on both faces.

Thank you for chiming in, Mike. As I said, striking errors are definitely one of my biggest weaknesses. This looked remarkably like the 2000 P in the link I provided for your site. How do you differentiate between the two? Added: I know they are in different directions but that should be irrelevant, I assume. A pic of yours and mine.

Makecents, your 1996 quarter is pictured with the reverse flipped from north to south. Since the offset on the obverse your coin moves from west to east, a north to south flip in position will leave the offset on the reverse face pointing in the same direction in the case of an uncentered broadstrike. In the Virginia quarter, the obverse offset is from northwest to southeast. If this had been an uncentered broadstrike, and the coin had been flipped from north to south, you'd see the offset design on the reverse pointing from southwest to northeast. Instead, the offset points from northwest to southeast, demonstrating that this is a dual misalignment.

Besides having a misaligned die it looks like a slot machine coin that has lost it's reeding due to excessive use and wear.
