Looks like PMD to me for a couple reasons: the line of damage extends to the rim and a little of the metal from the bottom of the letter D seems to have been "smushed" up into the middle of the letter.

Nope, it's not an error. It's Post Strike Damage (happened after it left the U.S. Mint). You can see the pushed and flattened metal.
