Looks like environmental damage .

How can you tell it's been cleaned? Is there any website that I could check out on cleaned coins.

It looks like someone tried to clean something off where the dark spot is, lightening the areas on either side.

Color is off for a copper coin, as well as the odd pattern where someone tried to clean off the dark color.
