So, I came across this 1942 quarter in my dad's collection and I was curious what on earth would have caused this?

Coins have been used and abused since they were made. There is not necessarily a logical explanation. When you have millions of coins circulating among millions of people, anything imaginable has been done many times. A whim, a science "experiment", a child with a whim and a hammer are 3 of many possibilities.

To me the interesting thing about this coin is that it is so badly damaged it's doubtful it was taken from circulation. What merchant would accept it? The damage was likely done by your father. If he had a diary you might actually scour it for the day he did it. And how it was done.
