Help 1995 D Penny Multiple Errors

I have been slowly researching and sorting through my inherited collection over the past few years and still have a long ways to go,but I am officially stumped. On this particular penny I see the D mint mark everywhere and I cannot identify the other markings and or damage.

@pf92, first welcome to CCF. Second, from what I can see of your pic, this cent just looks damaged to me (no mint errors). If the color is correct, then the copper plating may have been stripped off at some point.

Is there any way to get a larger, well cropped close-up photos of the areas in question, please? It's difficult to see the issue when it's so small. Anything you can do might help us identify what's going on. Thanks.

Yes. Large color, high resolution photos--obverse and reverse--would help immensely.
