So for those familiar with working at a mint, how do you propose an employee was able to 'get away' with this?

Just a couple of notes: The ANA Museum in Colorado has a 3 leaf clover of Proof Ikes on Nickels donated by Dr. A.K. Berry The State of Calif auctioned off a very large group of exotic mint state and proof U.S. error coins that were examined by the U.S. Secret Service, prior to the auction, and were authenticated as genuine and given the ok to auction them.
