I was wondering if any one can tell me if this is human error or production error?

Most certainly human error...or human intent....or parking lot error...but definitely not a minting error.

It is only an error if it happens during the striking of the coin and there
is no way that could have happened at that time.
just a badly mutilated coin.

Post Strike damage. A damaged coin is never worth more than face value. Spendable.
