Might have been clash lines on the dies? If so, the reverse could affected if they were the same dies when the clash happened?

the reverse does not seem to line up for a clash. the olive on the reverse should be completely over the lips and the leaf is fully on the chin, not on the field too. But that all said, I have no idea what did create this.

there is also an oval feature behind his ear. I guess with the condition of the coin it doesn't matter much, just wondering what caused the features.
