Found this quarter in a quarter roll and am unsure what to do with it. Save it or scrap it? What and how did this happen?

Just a damaged coin, probably done by a file.

Intentional extreme post mint damage. Appears to have been ground down. Indentation looks like damage from a BB shot. Spend it if you can. Value: 20 cents.
