Thanks for the extra images - Not sure what that is on either side of the 'S' of states. possibly, as mentioned a die gouge.

The reasons listed above are most likely one of what you have but I don't know that I would not send this in to Mad Die Clashes, when and if they are taking submissions again. The 90's were crazy for several different types of clashes.
