Was going threw some coins today and the D on this coin threw me off because it looks like the D was put on another D and squished it out of you look at the inner part of the left on the inside of the D you can also see that it looks like there is another D under it.

Dont know why the pics came out so small but the D looks like a good enough picture to tell.

The mint marks are part of the design since 1990. It can't be a RPM.

It is probably die deteriation. The die is falling apart from wear. But the image is so small it would help more with a larger image.
