I've had this for some time, but just rediscovered it in my stuff.

It's a zinc planchet with a weight of 2.6

What would be the proper term to describe this error? Off center?

Yes, looks to be about 95% off-center. Is the side in the second picture uniface(no design present)?

It looks like it. And in addition the planchet is extremely thin where it was struck.

No design from what I can see close up - and Yes, the outcrop is extremely thin.

Thoughts about grade/value?

off-center with indent.

It was struck off-center and there was another planchet in the coining chamber. So this piece was struck between the hammer die and a planchet instead of between the hammer and anvil dies.
