It's a cast fantasy loosely based on Kushan design.

I think it could be a religious medal like the brass anting-anting used in the Philippines. The Madonna and child are on one side in a formal pose between votive hanging lamps and on the other the kneeling form appears also to me Mary before a seated child like form of Christ. I will check my pictures of anting-anting to see if I can come up with a match.
