Large Raised "Bubble" On Washington Quarter

Was at the local coin shop today and they showed me an odd coin that stumped the person working.
The coin in question is a 1981 D Quarter. It does not appear to be uncirculated, but still has alot of detail. Just no luster.
There is a large raised bubble on the Mouth to forhead of Washington.
The reverse of the coin is normal, no indentation at all.
It is raised roughly to 2.5x times the normal thickness at that point.
Weight seemed normal 5.5g
