Tentatively-Identified-Greek-Death-Medallion

I've tentatively identified this as a death medallion.
It's in white metal 31mm.
Translated inscription
Obv: Cannes 30 March 1896 around a Chi Tau monogram.
Rev: Athens 11 April 1896 around a + sign with sprays of grain below.
Presumably the Chi-Tau monogram refers to CHARILAOS TRIKOUPIS, prime minister of Greece (1875 to 1895). He died in Cannes, France, on 30 March 1896. He was buried in Athens.

Is this identification correct, please ?
Can you tell me who the designer was ?
Can you provide me with a link to previous sale(s) ?
