What caused this? I am guessing strike through. Only half of the D is visible when the coin is tilted in certain lighting.

Looks to be Struck Through Grease, and I don't think that's a mint mark. Your third picture appears to be split plating of the "9" or some kind of bubbling below the "9", I'm no expert though.

The first picture shows the mm the best. It is hard to see unless the coin is tilted in the right light but is there and is close to the 9. There are some other raised areas below the date but I can't tell what would have caused them.
