Is this a Sasanian drachm of Khusru II ?

It's about 31mm in diameter and 4.09g in EF or better.
There's a bit of weakness in the strike or a bit of wear.

Where is the mint name, what is it, and how should I read it ?

Where is the year or date, what is it, and how should I read it ?

Approx retail value, too, please ?

Yes you are correct it is a Dirham (drachm) of Khusru II. Judging by the the dots at the collar and the crown type. I am not sitting in front of my Gobl right now but I recognize the type very well. I should be able to give you the mint and regnal year once I am back home. Approximate value of this coin is about $50 or so. He is the MOST common ruler of all.
