Fot the first one, my guess is a non metallic blank impurity (foreign body), perhaps a small stone or similar, which broke up when the coin was struck.

The 1999 circle is definitely post mint damage.

The 1968 is interesting though

What year has the Cud near the P'pus bill?

In my experience 1999 is the main year for rev Cuds in many places on the P'pus, plus on the 2 & 0.
