Anyone have any input on this 1964 nickel?

Looked like PMD to me but the reverse while as equally worn as the obverse doesn't not appear to show any damage if the obverse had been smashed with a hammer or something.

The obverse has horizontal lines running through it.

The coin is not bent and at no point does the area at 3 o'clock extend above the rim height obverse or reverse nor does it extend past the circumference of a regular nickel.

I think it is an error but I can't figure out what kind of error. Maybe a partial clip and a defective planchet or maybe a struck through but I really don't know.
