Many, from PCGS, NGC, ANACS, and ICG. I don't think I've seen one from SEGS. Last year in the Jan pre-Long Beach slae by the Goldbergs there was a group of 150 NGC slabbed Conder tokens. 10% of them were misattributed. The worst case was a common piece worth about $100 misattributed as a RRR (R-7 on the Sheldon scale) piece that hammered down for over $1250.
(Goldberg's had been notified of the misattributions before the sale, but afterwards they did advise that the buyers would be notified of the misattributions and they would be allowed to cancel the sales.)
PCGS for years had problems with 1794 head of 93 cents. Most PCGS "heads of 93" were actually Heads of 94, a difference in value of thousands of dollars.
I've seen fake 1804 large cents (altered dated) in both PCGS and NGC slabs.
So yeah the
TPG's do botch things sometimes, and often enough that you should NEVER trust an attribution from one of the
TPG's. Always confirm them yourself.
