At first glance, I don't see anything that makes it appear as a fake. However, these are very rare and in that condition very valuable. So from the price (if you're looking into buying it) you could maybe get an idea as to it's authenticity. Does it have a silver ring? Is it magnetic? These will also give you an idea as to it's authenticity, Silver isn't magnetic. Finally weight would further confirm it's authenticity although there are some very good silver fakes out there. What's that going on, on the bust?

From what I can see at first glance it´s a strike on an older ruble and the remains of the old bust is still there...

If genuine, this a very interesting and valuable coin as in 1742/43 many roubles of Ivan were over-struck with the image of Elisabeth. It should weigh 25.850g.
