Half of the edge is square, but the other half is thinner and not square at the edges.

Looking from the obverse, the Elizabeth II side is the square one, and the DG Regina is the round one.

Anyone knows what happened to it?

Sounds to me like a minting issue, perhaps something in the collaring process?

yes I agree with thedollarman and it is barely off center, keep looking and you will find another, they are fairly common!

Does being off-centre cause one side to be thinner than the other?

Maybe it might be a tapered planchet or have you ruled that one out ?

I don't know what a tapered planchet looks like and I don't really want to be making assumptions on mint errors.

A tapered planchet is one were one side of the coin is thinner than the other side (which is thicker),apparently its caused by too little pressure when rolling out the strip ....

the irony is that I came across this while investigating thin planchet coins and their causes ., I think I also got a tapered planchete 2p in change this summer ....
